Short Term Pathways for Support at Home
Getting help at home starts with the pathway that fits your situation right now.
Some people need ongoing, regular support; others benefit from a short burst of help, like Restorative Care, Assistive Technology & Home Modifications (AT-HM), or End-of-Life support.
The guide below shows how each short-term option works, how you access it, and what to expect next.

Restorative Care
What it’s for? A short, intensive burst of allied health and/or nursing to regain function and independence.
How do I access it? Approved as a short-term pathway in your Notice of Decision; care is coordinated by a dedicated Restorative Care Partner and a multidisciplinary team.
How long is it for? Up to 16 weeks, funded from a dedicated restorative care budget.
Do I pay a contribution? Yes, for independence/everyday living items; no for clinical supports.
What happens after? Exit planning starts early; you may continue or start ongoing services if needed, via support plan review/reassessment.
Assistive Technology & Home Modifications (AT-HM)
What it’s for? Funding for equipment and minor/major home modifications prescribed to keep you safe and independent at home.
How do I access it? Approved as a short-term classification in your Notice of Decision; includes an option for AT-HM when clinically indicated.
How long is it for? Time-limited funding specific to your prescribed item(s)/mods, guided by AT-HM scheme rules in the service list/guidelines.
Do I pay a contribution? Contributions can apply depending on the service category; clinical components are fully funded.
What happens after? AT-HM may be part of restorative or ongoing plans; your support plan is updated and can be reviewed if needs change.
End-of-Life
What it’s for? Flexible supports focused on comfort, dignity and staying at home wherever possible at the end of life.
How do I access it? Approved as a short-term pathway in your Notice of Decision; provider must meet End-of-Life Pathway requirements.
Maitland Community Care Services meets these requirements and will provide care with dignity and respect.
How long is it for? Time-limited classification with specific access and funding settings.
Do I pay a contribution? Contributions settings are defined in the pathway (clinical supports are fully government-funded).
