HOME CARE PACKAGES
WHAT IS A HOME CARE PACKAGE?
A Home Care Package (HCP) provides a coordinated package of services tailored to meet your specific needs. One of the benefits of having an HCP is the flexibility. Everyone's will be different. A Home Care Package can:
- help you stay in your own home as you get older
- give you choice and flexibility in the way your care and services are provided to you at home.
WHAT TYPES OF SERVICES ARE PROVIDED?
The types of services you get under a Home Care Package (HCP) will depend on your needs.
We will work with you to identify your goals and needs, to co-design the best ways to deliver those services so you can live independently at home and in your community.
A Home Care Package can include, but is not limited to:
- Personal care including showering and dressing;
- Household chores - help with the laundry, cleaning, meal preparation etc;
- Transport for shopping and medical appointments;
- Social Support visiting a friend, group activities i.e. movies, lunch, mystery outings,
- Personal safety monitoring devices & other equipment;
- Home modifications (e.g ramps, rails)
- Gardens and lawn maintenance
- Wellbeing programs inc gentle exercise group, social outings, concerts, lunches, movies, mystery outings etc.
- Connect with Tech - getting and staying online safely, keeping you connected with family and friends
- In-home nursing, physiotherapy, occupational therapy and other clinical services
- Care Coordination and case management
You can also use your HCP to pay for your regular care needs while you are away on holiday (but not to pay for your holiday). And if you don’t use one or more of the services in your package for a period of time, because you are away or in hospital for example, you may be able to use or save some of the funds for other things.
There are some things you CANNOT use your HCP for, including any item or adaption that would usually be paid for out of your general income.
A Home Care Package cannot be used to pay for:
- home improvements
- home appliances or fixtures
- domestic appliances
- your rent or mortgage
- buy your groceries
- your holidays
